Overview

This CPD module is from our MSc Advancing Practice course.

This module has been designed for health, social care and allied health professionals, irrespective of speciality or place of work, who want to enhance their knowledge and gain an in-depth understanding of long term conditions. You’ll explore:

  • national and international policy contexts of living well with long term conditions and multi-morbidity
  • risk factors and illness trajectories
  • concepts of self-care and self-management
  • models of care
  • service delivery and building care partnerships
  • developing professional roles and responsibilities

The long term condition remains undefined but where appropriate the module refers to many conditions for which there is currently no cure, and which are managed with drugs and other support and treatment options. This may include conditions such as depression, diabetes, chronic obstructive pulmonary disease, arthritis, hypertensions, with many people living with one or more of these.

Entrance requirements

Current professional registration with NMC, HCPC, GPC or SSSC is required for entry to this module. Applicants are required to have a degree qualification.

English language requirements

If English is not your first language you must have one of the following qualifications as evidence of your English language skills:

  • IELTS Academic or UKVI 6.0 with a minimum of 5.5 in each sub-skill.
  • Pearson Test of English (Academic) 56 overall with a minimum of 51 in each sub-skill.
  • IBT TOEFL 78 overall with a minimum of 17 in listening, 18 in reading, 20 in speaking and 17 in writing.

Funding

A flexible payment scheme is operated by the University for students who are self-funding.

Objectives

Completion of the module provides opportunity to attain the following Learning Outcomes:

  • Describe key national and international policies and priorities in relation to the care and management of people living with long term conditions and multi-morbidity.
  • Identify and review relevant physical, psychological, behavioural, and socio-economic issues relating to individuals and families living with a long-term condition(s) and its treatment.
  • Critically appraise relevant models of care and service delivery.
  • Synthesise the evidence base relating to the self-management of people living with long term conditions in relation to a chosen practice and/or focus.
  • Critically reflect on the role of healthcare professionals in supporting people with long term conditions, including responsibilities and collaborations.

Structure and content

The module is divided into four units:

  • Unit 1: Introduction to long-term conditions.
  • Unit 2: New Models of Care.
  • Unit 3: Supporting People with Complex needs.
  • Unit 4: Professional roles, responsibilities, skills and knowledge.

Employability

All modules can be linked to specific professional capability/competency frameworks within your practice area. You are encouraged to view your participation within the MSc Advancing Practice programme as a means of supporting your professional development plans (PDP) and career progression.

This module may be completed as a single credit-based module. Alternatively, you can use it as credit towards the MSc in Advanced Practice.

Modules will be offered subject to demand and need to meet a minimum class size of eight students.

Career opportunities

Completion of the Advancing Practice course provides an opportunity to develop transferable skills. This will optimise the ability of each student to impact the experience of individuals, families and service providers within a range of clinical, social and pastoral settings within the UK and internationally.

All modules are related to NHS Knowledge and Skills Framework (KSF), enabling you to clearly identify the core and specific dimensions within the KSF which can be achieved when you successfully complete your module/programme.
